- To apply you must meet the guidelines listed below:
- At least two (2) years verifiable experience as a care provider
- Proof of Legal authorization to work in the United States
- High School Diploma or Equivalent
- Clean Background
- Current CPR
- Current TB Test or Chest X-Ray
- Driver’s License
- Reliable transportation & auto insurance
- You must bring with you all of the documents listed below:
- Proof of Legal authorization to work in the United States (Social Security Card, Passport, Green Card, etc)
- Last five years of work history – names, addresses, phone numbers
- Three personal references (not family) – name and phone numbers
- $20 for a National Criminal Background Check to be performed
- Your CNA, LPN, or RN license (if applicable)
- Current CPR
- Current TB Test or Chest X-Ray
- Driver’s License
- Proof of auto insurance
- You should print the application at the bottom of this page,complete it and fax it to 404-252-9035 (Georgia) or (205) 870-8866 (Alabama). When we are accepting registrations, you may bring the original to our office for your appointment.
